Fillable forms download w errors; print to pdf and it crashes Kofax Power PDF when opened

My default pdf application is Kofax Power PDF 3.1. Windows 10 Pro. I have used a particular fillable form online for years and downloaded and printed to pdf all day with no problems.

When I download the form after completing it online, certain text fields (month name, county, etc.) are changed to numerals during download. For example, the form requires a date that is entered as ______ (day of the month) ______ (month name spelled out, selected from drop down menu), ____ (year), etc.. The form online shows "February" in the month field, but after the form is downloaded, it shows up as "02".

When I print to pdf, the application crashes w no error message when I attempt to open the pdf.

If it is Firefox that crashes, then if you open the page `about:crashes` and click on the links there it should take you to your crash reports. You can then open a bug report at https://bugzilla.mozilla.org/ with links to your bug reports.
