Right click on the empty space of any other toolbar (e.g. Bookmarks Toolbar) and a little menu appears. Click on Menu Bar to enable the toolbar you're missing.

You can also try to reset the Firefox plist file and repair the disk permissions.

Go to "Library > Preferences" and remove the plist file for Firefox (org.mozilla.firefox.plist).
