Private browsing is off, remember passwords is checked, site is not in the exceptions list, but I am not asked if the password should be saved. The password for the site had been saved until I was forced to change it. I deleted all references to the site in the saved passwords list, but still do not get the prompt.

The website may be using autocomplete=off to prevent Firefox from saving the name and password.

You can remove autocomplete=off with a bookmarklet to make Firefox save the name and password.

I got around it by forcing an "invalid user id or password" message from the site. When I keyed the correct password, the "Remember Password" box popped up.
