Firefox Pinned Websites/Frequent Websites Have No FavIcon

When I first starting using Firefox, everything was working normally. Then over the past month I've noticed that all icons now show a generic icon for all frequent websites and pinned websites. I've tried "favicon refresher" addons but none work. Plus, I've tried to delete and re-add the websites to the pinned list, but that fails to work as well.

You can check for problems with the places.sqlite database file in the Firefox profile folder.
