A blank square appears at the top left corner of my screen after close the preferences window in Mac OS X
Whenever I open the preferences window and close, a blank square of 120px (aprox) appears at the top left corner of the screen. The square doesn't dissapear untill I exit from Firefox and reopen it.

This issue can be caused by an extension that isn't working properly.
FoxTabs has been reported to cause this issue.
Start Firefox in Diagnose Firefox issues using Troubleshoot Mode to check if one of the add-ons is causing the problem (switch to the DEFAULT theme: Tools > Add-ons > Themes).
- Don't make any changes on the Safe mode start window.

cor-el, the problem continues. In addition, the preferences window appears off screen the first time that opens.
Good news for me, but the cause of the problem is still a mistery. I uninstalled Firefox, reinstalled it and all the extensions that I was using and the problem has gone away.
Now, I've also realized that there weren't top buttons in the preferences window (close, minimize, maximize)

I found that by turning on the sticky menu bar the problems with the top of the screen acting strange disappeared. In my case it was overwriting the Minimize/Close with a black streak. Now it does not.
Firefox 5 Beta also suffers from this issue.
In doing a search for info on this issue in FF4, I did find something about creating a new profile and the trying to recreate the problem and on the new profile the problem does not occur.
Unfortunately this problem is keeping me from moving to new version. Moving everything to new profile is more work than I want to go through.
Safe mode as others have said does not solve the problem.
