You can do that from

Firefox Menu > Print Setup (Press the right arrow button in the right end of print option) > Print setup > Margin & Header/Footer tab

In Header/ Footer section select --blank--- for from drop down for everything or which ever you don't want

Under the File menu, select Print Setup. The Margin/Header & Footer tab will let you customize or delete the header.

Mac users need to access the appropriate preferences thru about:config

print.print_header ----
print.print_footer ----

There's left - center - and right prefs for each, on Windows and Linux. Probably the same for Mac.
