Flash video is not playing, audio is fine

Flash video is not playing on my website, but I can hear the audio perfectly fine. The video player's interface is not visible either - blank space instead.

I'm using Windows 7, Firefox 42.0 (also tried 43.0), Adobe Flash 20.0.0.235. I've seen some similar questions here and here's what I've tried:

- restarting and reinstalling the browser;

- clearing cookies;

- using safe mode;

- disabling hardware acceleration.

Nothing helped. Is there anything else that can help?

Ok, after digging deeper into my problem and testing in different environments I realized that it's not flash or firefox issue. The video player was inside of a bootstrap modal and this one - http://stackoverflow.com/questions/23709700/embed-tag-inside-bootstrap-modal-not-visible-in-firefox - helped me.

Updated FF to 43.0, it didn't help.

Do you have an example, that would help test the format or get an example into a bug with Adobe or bugzilla once we figure out which one :-)
