How can I prevent my custom toolbar from disappearing?

My custom toolbar (I call it "Utility Bar") has disappeared from my browser 5 times in the last 2 weeks. It is not listed in View/Toolbars flyout menu. It just completely goes away. I rebuild it from scratch and soon it will go away again. This is quite annoying.

That sounds like a problem with the file localstore.rdf.
