not prompting for or saving passwords
I'm using 40.0.02 and Firefox is no longer prompting to save passwords, and all of my previously saved passwords are gone.
Settings: - Under Privacy - Firefox will remember history - Under Security - the box is checked to remember passwords - and there are no exceptions
One suggestion is to refresh Firefox - I did that once before to solve a problem and everything got kerfuffled, so I'm hesitant to try that.

Start Firefox in Safe Mode to check if one of the extensions (Firefox/Tools > Add-ons > Extensions) or if hardware acceleration is causing the problem.
- Switch to the DEFAULT theme: Firefox/Tools > Add-ons > Appearance
- Do NOT click the Reset button on the Safe Mode start window

It is possible that there is a problem with key3.db (encryption key) and logins.json (encrypted names and passwords) in Firefox. You can open the logins.json file in the Scratchpad (Firefox/Tools > Web Developer) to check its content. Click the "Pretty Print" button to format the file for readability. If there are names and encrypted passwords in logins.json then there is likely a problem with the key3.db file that stores the encryption key. You can rename the key3.db file (key3.db.old) and logins.json file (logins.json.old) or move these files to another folder to make it possible to undo the action. Rename or delete a possible signons.sqlite file (signons.sqlite.old) used by older Firefox versions as well. You need to set a new Master Password after renaming or deleting key3.db and all currently saved passwords are lost. If that has worked then you can remove the renamed files that are no longer needed.
You can use this button to go to the current Firefox profile folder:
- Help > Troubleshooting Information > Profile Directory: Show Folder (Linux: Open Directory; Mac: Show in Finder)

Thanks for your help! I ended up disabling extensions one by one until I found the culprit! Ironically, it was a 'remember password' extension I'd added months ago to address the same problem...

Yes, the "Remember Password" extension has been reported to cause issues with the passwords disappearing. You didn't include the extensions list in your post, otherwise we could have told you right away.
You do not need this extension because current Firefox releases ignore autocomplete="off" when saving a password.
