Updated to Firefox 4b11, now I have two title bars!
I just upgraded Firefox 4, from beta 10 to beta 11, and now it's showing both the "Windows XP" title bar and the "Windows Vista" / "Windows 7" title bar. Here's a screenshot: http://i.imgur.com/araTf.png
Not that it bothers me but it looks weird :)
UPDATE: Solved it by hiding the menu bar and showing it again, via right click on the menu bar. Now it shows just one depending on whether the menu bar is hidden or not, but that first-time bug needs to be fixed :P

I have the tab bar always showing, and I had to open a new tab first before walenzack's fix worked.
edit: It looks like this problem is due to either a corrupted user profile, or Firebug. The version of Firebug I'm using is "firebug-1.7X.0a10.xpi" and that seems to break everything. Using an older version, a8, seems to be fine.

What extensions are you using? There are reports that this is caused by Firebug and/or Stylish but I'd like to confirm or find out if there are other extensions that cause this.
Disabling Firefox fixes the issue. Anyway, in Linux (Fedora) I don't have the problem with the same extensions, just in Windows XP.
This happened to me and a colleague as well after installing Firebug 1.7X.0a10. Furthermore you loose your tabs and other settings like browser layout.
Disabling Firebug fortunately restores everything. Re-enabling Firebug messes everything up again.
A little looking around, and I found out it's reported in (and fixed in next release of) Firebug. See comment #24 for a workaround for the current release.
I am not using either Firebug or Stylish, but get this problem when opening a new window (the first window doesn't have this problem). Also, when I tear off a tab to a new window, I get the two title bars, the torn-off tab remains in the original window, and the tab in the new window is empty. Very frustrating.
Start Firefox in Diagnose Firefox issues using Troubleshoot Mode to check if one of the extensions is causing the problem (switch to the DEFAULT theme: Firefox (Tools) > Add-ons > Appearance/Themes).

In Firefox 4 Safe mode disables extensions and disables hardware acceleration.
- Tools > Options > Advanced > General > Browsing: "Use hardware acceleration when available"
If disabling hardware acceleration works then check if there is an update available for your graphics display driver.
