When you start typing in the Smart Location Bar, a drop-down list of pages from your history and bookmarks that match what you've typed will be shown. The Smart Location Bar will actually learn based on your selections.
For example, if you type g and consistently pick google.com, then google.com will become the first in list every time you type g in the Location bar. Training the Smart Location Bar in this way for a few days will greatly improve its accuracy in predicting what you want.
Why clearing history does not clear the Location bar
Items that appear with a star along the right edge are part of your Bookmarks and not your browsing history.

Removing bookmarks results from the list

- Select the Privacy panel.
- Beside When using the location bar, suggest, set the drop-down menu to either History or Nothing.
- History will make the Smart Location Bar only display results found in your browsing history, not bookmarks.
- Nothing will turn off autocomplete.

To prevent bookmarks from appearing in the list, you need to install an add-on called Hide Unvisited. Hide Unvisited is an extension that will remove any page that you haven't visited from the list of results.
